When submitting plans for listing, we need a submittal form to accompany it.
If you are submitting a hand written form, you may wish to print the PDF version and fill it in. If you have a recent version of MS Word (97 or later), you may download the Submittal template. Save it to a convenient folder. When you open it in MS Word, it allows you to fill in the blanks, using the Tab key to advance to each option, and then perform a "File, Save As" to save the completed document .DOC file as a unique name which can then either be submitted electronically or printed for mailing or faxing. If you do not have MS Word, you may be able to open the .DOC file directly in your browser, fill in the blanks, using the Tab key to advance to each option, and then either Print or Save the file to your computer. You can not just fill in the blanks within the browser and do a "File, Send, Page by Email", as this will just send a blank template. The Saved document may be emailed to the appropriate address. |
